Built on a foundation of trust, Southland Care Coordination Partners, Inc. (SCCP) was formed to effectively address the healthcare and social service needs of residents of south suburban Cook and Will Counties. Under the guise of both federal and state health care reform mandates, SCCP operates as Care Coordination providing comprehensive care management services for eligible residents and healthcare providers. Through its leadership and partners, SCCP knows the Chicago Southland. We work and live in this community and intimately know the challenges that prohibit many from access to quality healthcare. And we know that health means more than just our physical health, but rather includes our mental, social, economic, psychological and spiritual health. This integrative force provides the foundation of why we exist. It is our mission to create a culture of health that embraces innovative community-centered practices that empower the individual and community at large.
